DTC P0420

A few years ago, on my aging 2001 Subaru Impreza 2.5 RS, I got a check engine light. With the Actron CP9175 OBD II AutoScanner, I pulled the following code:

P0420
Catalyst Efficiency
Below Threshold
(Bank 1)

I cleared it back then, and it comes back once in a while. But early this month, it started coming back all the time after clearing it. Looks like whatever worn out has finally failed. What could it be?
